FINAL PAPERWORK NIGHT IS SCHEDULED FOR JULY 15TH FROM 6 TO 8 PM IN THE CAFETERIA AT SHAKER HIGH SCHOOL. PLEASE HAVE THE FOLLOWING WITH YOU:
1. Recent photo of your (head and shoulder shot) child.
2. A copy of your child's birth certificate.
3. Physician's statement signed by doctor.
4. Player/parent contract (blue card).
5. Copy of your child's end of year report card.
